Our Mission
…is to work together to improve diabetes and heart health outcomes by using consistent messages, developing educational resources, and advancing best practices to support healthy behaviors in the community.
How We Work
Since 2000, the MN-DC has been a powerful partnership with a collective reach 80-85% of people with diabetes in the state, as well as most primary care providers, local public health agencies, senior centers, home care nurses, and many others. This reach makes the MN-DC an unrivaled communications team.
